There are 2 versions of each form.


Firstly a PDF file for which you need Adobe Reader (see below) You can print out this PDF file and then fill in your details by hand and attach the completed form to your images.
Secondly there is a Word Document which you can download, save and fill in on your PC, then print out and attach to your images.
There are also PDF and Word Document Files for Labels to attach to the back of your prints.
